The ICD-10-CM (International Classification of Diseases, 10th Revision, Clinical Modification) provides a standardized avenue for classifying and recording diseases and medical conditions.

Accurate diagnosis coding leads to better communication across the care continuum and ensures healthcare providers receive timely and accurate reimbursement for their services. ICD-10-CM codes are also used for data collection and public health research. ICD-10-CM data are used to monitor disease trends, track disease outbreaks, and aid in the allocation of resources.

Healthcare organizations participating in value-based initiatives use ICD-10-CM data to track quality measures and identify areas for improvement. They also use ICD-10-CM codes to communicate the complexity of patient care and identify patients who may be vulnerable to future hospitalizations and other serious health events.

Accurate ICD-10-CM coding supports the medical necessity of services provided and assures compliance with legal and regulatory guidelines. Failure to code correctly can lead to improper billing, audits, penalties, and even legal action for healthcare providers. Detailed documentation is required to support the ICD-10-CM codes submitted to Medicare, Medicaid, and other third-party payers.

2025 Chapter Updates

Chapter 1: Infectious and Parasitic Diseases (A00-B99) No significant changes other than a spelling correction for Ehrlichiosis chaffeensis

Chapter 2: Neoplasms (C00-D49) 63 new codes, particularly for lymphomas, with new codes indicating whether the lymphoma has not achieved remission or is in remission

Chapter 3: Diseases of the Blood and Blood-Forming Organs (D50-D89)

A new code for Fanconi anemia

Chapter 4: Endocrine, Nutritional, and Metabolic Diseases (E00-E89)

16 new codes have been established relating to conditions for presymptomatic type 1 diabetes mellitus, hypoglycemia levels, and obesity classifications

Chapter 5: Mental, Behavioral, and Neurodevelopmental Disorders (F01-F99)

26 new codes have been established for reporting the severity of eating disorders such as anorexia, bulimia, and binge-eating

Chapter 6: Diseases of the Nervous System (G00-G99)

7 new codes related to KCNQ2, a neonatal-onset epilepsy syndrome, and other epilepsies and disorders of the autonomic nervous system

Chapter 7: Diseases of the Eye and Adnexa (H00-H59)

No new codes, however, minor revisions to the wording for degenerative myopia have been made

Chapter 8: Diseases of the Ear and Mastoid Process (H60-H95)

No changes for 2025

Chapter 9: Diseases of the Circulatory System (I00-I99)

4 new codes have been added for cement or fat embolism of the pulmonary artery

Chapter 10: Diseases of the Respiratory System (J00-J99)

7 new codes for nasal valve collapse

Chapter 11: Diseases of the Digestive System (K00-K95)

27 new codes for anal and rectal fistulas, classified by site and type

Chapter 12: Diseases of the Skin and Subcutaneous Tissue (L00-L99)

8 new codes for conditions like cholestatic pruritus and specific types of alopecia

Chapter 13: Diseases of the Musculoskeletal System and Connective Tissue (M00-M99)

33 new codes for musculoskeletal conditions, such as disc degeneration, synovitis, and tenosynovitis

Chapter 14: Diseases of the Genitourinary System (N00-N99)

No changes for 2025

Chapter 15: Pregnancy, Childbirth, and the Puerperium (O00-O9A)

No changes for 2025

Chapter 16: Certain Conditions Originating in the Perinatal Period (P00-P96)

No changes for 2025

Chapter 17: Congenital Malformations, Deformations, and Chromosomal Abnormalities (Q00-Q99)

4 new codes for congenital malformations of the aortic and mitral valves

Chapter 18: Symptoms, Signs, and Abnormal Clinical and Laboratory Findings, Not Elsewhere Classified (R00-R99)

1 new code for anosognosia

Chapter 19: Injury, Poisoning, and Certain Other Consequences of External Causes (S00-T88)

30 new codes related to adverse effects of immune checkpoint inihibitors (ICI)s, immunotherapy, and gastrointestinal tract anastomosis disruptions

Chapter 20: External Causes of Morbidity (V00-Y99)

No significant changes

Chapter 21: Factors Influencing Health Status and Contact with Health Services (Z00-Z99)

25 new codes related to social determinants of health, pediatric obesity, family history of colon polyps, and sepsis aftercare encounters

Chapter 22: Codes for Special Purposes (U00-U85)

No changes for 2025

Summary
